Conference registration is now OPEN!  Please use the appropriate form when registering. Cancellation and payment policies are noted below.  It is very important to provide an accurate email address for each registrant, as this will be used to create a unique login for the conference.

PWDA Members - $450
Non-members $525 -  Not a member?  Click here to join!

Registration Form - use this form if you are not a state agency employee. PWDA Champion, Partner and Individual members, if you are unsure of your member discount code, please contact staff at [email protected]

State Agency Form - use this form if you are a state agency employee of the following state Departments: Agriculture, Corrections, Community & Economic Development,  Education, Labor & Industry, and Human Services. If you have a question about whether or not you should use this form, please contact staff at [email protected] 

Payment Policy
PWDA accepts credit card and check payments.  If you select to pay by check,  all payments must be received in the PWDA office no later than April 14, 2021. By electing to pay by check, you commit to making payment for your registration. Cancellation or no show without payment does not exonerate you from any fees due. 


Cancellations/Refunds/Substitutions
A refund, minus a $100 administrative fee, will be given only if a cancellation is submitted in writing to [email protected] by April 5, 2021. Refunds will not be given after this date. Any revisions made by PWDA staff (including name changes) after this date will incur a $100 administrative fee.
